For Veterinarians
Heartland Veterinary Pharmacy accepts prescriptions for compounded medications by fax or phone only.
Prescriptions must be issued by a licensed veterinarian and indicate a valid patient-client-veterinarian relationship (VCPR).
How to Submit a Prescription

To process prescriptions accurately and promptly, please include the following information:Veterinarian & Clinic
- Veterinarian name
- Clinic name, address, phone, and fax
- Veterinarian license number
- Veterinarian signature
Patient & Client
- Owner name and address
- Patient name
- Species
- Date of birth, weight, and gender
- Known allergies
- Current medications
- Relevant medical conditions
All compounded medications are prepared pursuant to a valid veterinary prescription.
